VERIFICATION FORMS
- Do I have to complete a Verification Form if I have already been certified and received money from the Phase II Tobacco Trust and/or the Tobacco Transition Payment Program ("TTPP" or "Buy Out")?
Yes. To be eligible to receive money under Phase I, you must make sure all pre-printed information contained in the Verification Form is correct, make any needed corrections, obtain any necessary documentation and sign and return the Verification Form by the deadline (May 19, 2010) to Troutman Sanders LLP's Tobacco Indemnification Program at P.O. Box 1198, Richmond, Virginia 23218-1198. If you do not complete the form and return it by the deadline, you may not receive money this year from the Commission. The Phase I Program is separate from the Phase II Program and TTPP. Therefore, you are able to receive payment from each program if you are eligible.
- I grew tobacco in 1998 but have not grown tobacco since. Am I
eligible for payment this year from the Phase I Fund?
Yes. The Commission has decided that producers who raised tobacco in 1998 will be eligible for producer payments this year even if the producer did not grow tobacco after the 1998 season.
- I held quota as of July 1, 1999 but have since sold my quota. Am I still eligible for payment this year from the Commission?
Yes. Flue-cured and Burley quota owners who held quota as of July 1, 1999 will be eligible for payments from the Commission for this year.
- I recently moved and my old address is listed on the Verification
Form. What do I need to do?
A change of address may be made directly on the Verification Form. We
strongly encourage you to notify the FSA Office about the changed
information so that your records will remain current.
- My Verification Form does not include a W-9 asking me to verify my
Social Security number. Why?
If you already provided a correct W-9 certification, federal law does not
require you to certify that number again. We will, however, contact you if
the IRS notifies us that the number you have provided is incorrect. Claimants
who have not previously provided a W-9 certification will receive one in 2010.
If you fail to complete your W-9 certification form, we are required by law to
withhold taxes on any monies you receive under the Phase I Program.
